FLU CLINICS

Flu shots are available at the Health Department on a walk-in basis Monday- Friday 9 am to 4 pm. Suggested donation is $15. No one will be turned away for inability to pay. Bring your Medicare card, Medicaid, or insurance card as some insurance policies will cover the cost.

Seasonal Influenza ("Flu") vaccinations are offered throughout Polk County from October through November at locations and times to be announced. 2011 Public Seasonal Flu Clinic Schedule To speed up the process at the clinic, please print and fill out the following consent form and bring it with you to the clinic.
