HIV OUTREACH PROGRAM |

The primary purpose is to provide intervention and conduct health
education/risk reduction and intervention for targeted populations. At risk populations
have been reached by accessing shelters, substance abuse facilities, prisons, parks,
schools and street outreach. Interaction with clients is specific to the population
and individual need.

Interventions include one to one interactions, group level counseling community
level programs and outreach. Each outreach worker focuses on behavior modification.
The goal for individuals includes but is not limited to reducing risk for HIV and
other STD..'s. Clients are provided with incentives, education, resources and supplies.

All resources provided by The Polk County Health Department have been used to support
outreach.

The second purpose is HIV testing. Sites where at risk individuals
congregate have been selected. These sites include an extensive list of places to
meet the needs of a variety of clients. Pre and post test counseling is provided,
along with education and prevention materials.
