GREEN CARD (PERMANENT RESIDENCY) IMMUNIZATIONS |

The purpose of Green Card ("Permanent Residency") Immunizations is to ensure that immigrated persons are up to date on immunizations for vaccine-preventable diseases. Green Card immunizations are done on Tuesday from 8-10 am and Friday from 1:30-3:30 pm by appointment at 286-3798. Cost is dependent on vaccinations needed. Persons should bring their immunizations records to the appointment.

Vaccinations required for Green Card depend upon birth date:
- Birth to 17 years
- Diphtheria/Pertussis/Tetanus
- Polio
- Measles/Mumps/Rubella
- Varicella
- Hepatitis B
- Haemophilus/Influenza Type B
- Adults born in or before 1956
- Tetanus/Diphtheria (one within the last 10 years)
- Varicella (2 at least 30 days apart)
- Adults born after 1956
- Tetanus/Diphtheria (one within the last 10 years)
- Varicella (2 at least 30 days apart)
- Measles/Mumps/Rubella (2 at least 30 days apart)
- Adults age 65 and over
- Tetanus/Diphtheria (one within the last 10 years)
- Varicella (2 at least 30 days apart)
- Influenza (recommended yearly)Influenza Vaccine
- Pneumococcal - "pneumonia" (depending on risk factors)
