GREEN CARD (PERMANENT RESIDENCY) IMMUNIZATIONS

The purpose of Green Card ("Permanent Residency") Immunizations is to ensure that immigrated persons are up to date on immunizations for vaccine-preventable diseases. Green Card immunizations are done Monday and Tuesday from 8:00a to 3:30p and Thursday and Friday from 1:30p to 3:30p, by appointment at 286-3798. We are able to complete the I-693 form for those needing vaccinations only. We are unable to do the I-693 physicals. Cost is dependent on vaccinations needed. Persons should bring their immunizations records to the appointment.

Vaccinations required for Green Card depend upon birth date:

DT, DTP, DTap

Ages 2-6 Years

TD, Tdap

Ages 7 Years and up (A total of 3 vaccines)

Polio, IPV or OPV

Ages 2-17 Years

MMR ( Measles, Mumps, Rubella)

Ages 12 mo and up (Ages 4 years and greater will need a series of 2 vaccines those refugees born before 1956 do not need the MMR)

Ages 2-4 Years

Hepatitis B

Birth to 18 years (A total of 3 vaccines)

Varicella (chicken Pox)

12 months and up or history of disease (A total of 2 vaccines)

Pneumococcal Vaccine Prevnar

According to recommendations, Ages 2 months to 23 months pneumovax 65 and older

Influenza Vaccine (recommended yearly)

Ages 6 months and older

Rotavirus

Children 6 weeks to 32 weeks

Polio, IPV or OPV Ages 2-17 Years

Ages 2-6 Years

Hepatitis A

Children 12 mo to ages 23 mo. (A 2 shot series)

Menigococcal

Ages 11 to 18 Years
