The Polk County Health Department has a team of 3 nurses that follow up on
all reportable diseases. Follow up may or may not include
telephone interviews, written CDC forms, referral, lab work, and/or treatment.
Contacts to an infectious disease may or may not qualify for lab testing and/or
preventative treatment. Persons with questions or concerns may call for
information. Some examples for reportable infectious diseases are: tuberculosis,
hepatitis, and meningitis.
