Seasonal flu Information
Seasonal Flu vaccinations (mist and shots) are available at Polk County Health Department, 1907 Carpenter Avenue (Monday-Friday, 9am-4pm ). A $15 donation is suggested. Medicaid, Medicare Part B, Wellmark and Humana will pay for the cost in full. Bring your insurance, Medicare or Medicaid card.
