January 21, 2021

Starting, Thursday, January 21, 2021, The Polk County Health Department and select Hy-Vee and Medicap pharmacies will open COVID-19 vaccine appointments to 65 years and older pending vaccine allocation at their pharmacy locations. Partners will continue to vaccinate Phase 1A during this time.

February 1, 2021, COVID-19 vaccine appointments will be open to:

• 65 years and older
• First Responders
• PK-12 staff, early childhood education, childcare workers
• 1A: Healthcare workers and long term care facilities staff and residents

Healthcare workers eligible for vaccination must meet the following criteria:

• Healthcare personnel (HCP) with direct patient contact and thus who are unable to telework, including those who work in inpatient, outpatient, or community settings, who provide services to patients or patient's family members, or who handle infectious materials

• Health care personnel are defined as paid and unpaid persons serving in health care settings who have the potential for direct or indirect exposure to patients or infectious materials.

Vaccine is available at the several locations in Polk County.  ALL locations will require that healthcare workers bring the following to their appointment:

• Mask: everyone will be required to wear one. If you do not have one, a mask will be provided to you.
• Photo form of personal identification: driver’s license, passport, or state identification card AND employment verification: employer issued badge or identification or proof of licensure.
• If you are pregnant or lactating, you are required to consult with your Primary Care Physician before-hand and come with a note from them stating you are ok to receive the vaccine.
