Polk County, Iowa desires to promote and provide for the public health, safety and welfare of its citizens. To further this public purpose Polk County is offering a promotional opportunity to Polk County residents.
Beginning Tuesday, June 15, 2021 through Sunday, August 22, 2021 at midnight, Polk County will offer a vaccine lottery to help boost COVID-19 vaccination rates in the county. Individuals are eligible to register after receiving at least one dose of the COVID-19 vaccine (1 dose of either Pfizer, Moderna or Johnson & Johnson). Once individuals are fully vaccinated (2 doses of Pfizer/Moderna; 1 dose of Johnson & Johnson), they will receive a cash prize if chosen. Residents of Polk County, Iowa, are eligible to register to enter the lottery and win prizes.
Polk County will verify (To confirm) that registered participants (an individual is) are fully vaccinated, (we will) by reviewing registered participants (your) vaccination records (on) through Iowa Immunizations Registry Information System. There is no cost to receive a COVID-19 vaccine and there is no purchase necessary to enter the lottery. Individuals entering the vaccine lottery must be 18 years of age or older. If you are 12 - 17 years old, you are eligible to enter for a scholarship. IF YOU ARE BETWEEN THE AGES OF 12-17 AND REGISTERING FOR THE SCHOLARSHIP PLEASE FILL OUT THE PHONE NUMBER AND/OR EMAIL OF YOUR PARENT OR LEGAL GUARDIAN.
When registering for the lottery, (individuals) participants will be required (need) to provide a valid Polk County address, name, date of birth, phone number and email address. If (your) the required information is not valid, (you) the registrant will be removed from the lottery and ineligible to win. Winners will be chosen by chance. Participants are not required to be present in-person at the time the winners are determined. Winners will be contacted via phone or email.

Q: What is the prize?
A: Starting on June 25 weekly cash prizes will be drawn. Every Friday ten $1,000 prizes will be drawn. Every other Friday one $50,000 prize and one $5,000 scholarship will be drawn in addition to the ten $1,000 prizes. The schedule for drawings for the $50,000 prize and $5,000 scholarship is June 25, July 9, July 23, August 6, August 20 and a possible bonus drawing on August 27.
Q: Who is eligible?
A: Any Polk County resident who is 18 years of age and older and has received at least one dose of a COVID-19 vaccination is eligible to register. Only those who are fully vaccinated will be eligible to collect cash prizes. If you are receiving a two-dose vaccination you can register for the lottery after receiving the first dose. If your name is chosen, you will not be eligible to collect your winnings until after you receive your second does. Polk County employees are eligible, except for elected officials and their immediate family members.

Q: How do I enter?
A: Go to https://vaccinelottery.polkcountyiowa.gov/ and answer several basic questions.
Q: What if I can’t register online?
A: Paper registration forms are available at all Polk County senior centers. See a list of senior center locations here (https://www.polkcountyiowa.gov/community-family-youth-services/senior-services/locations-and-activity-calendars/). You can also call our COVID-19 call center at 515-323-5221 for assistance registering. The call center is open Monday - Friday from 9:00 AM to 4:00 PM.
Q: Am I automatically entered if I have been vaccinated?
A: No. You must actively register to enter the lottery.
Q: How many times do I need to register for the lottery?
A: You only need to register once.
Q: Can I still enter the lottery if I was vaccinated in another state?
A: If an individual was vaccinated in a state other than Iowa, they will need to show proof of vaccination (COVID-19 vaccination card) to receive a prize.
Q: What if I entered my information in wrong? Can someone re-enter the information?
A: Unfortunately, we are unable to re-enter your information, if you have made an error on your entry form. Please remember to check that all of the information you provided is correct BEFORE submitting your entry. If your name is selected, you will need to show verification of age and address (driver’s license, passport, pay stub, etc.)
Q: Do I have to receive my vaccination at the Polk County Health Department?
A: No. You can receive your COVID-19 vaccination anywhere. Only residents of Polk County are eligible but you can get vaccinated anywhere you choose. Health Department staff will verify full vaccination status for winners using the state vaccination database. Free vaccinations are available at https://www.immunizepolk.com/
Q: What if I’m already fully vaccinated?
A: Thank you for doing your part to protect our community! You are also eligible for the cash lottery and can register as noted above.
Q: Will my name be released to the public if I win?
A: Winners will be asked to sign a waiver of release as the names of winners are subject to public record. The pool of eligible individuals who have registered for the lottery will not be made public.
Q: How are winners selected?
A: An auditing firm will be hired to conduct random drawings. Two lists will be generated each week from the pool of registrants, one for those over 18 who are eligible for the cash drawings, and one for those under 18 who are eligible for the scholarships. Staff will verify age, Polk County residency and vaccination status of all names that are randomly drawn.

Q: Are Polk County tax dollars being used to pay for this?
A: No. Polk County tax dollars are not used for incentive prizes. The incentive program is funded by American Rescue Plan Act (ARPA) dollars. These are federal funds provided to local governments to assist in the distribution of COVID-19 vaccine and recovery of our community.
Q: Are the lottery winnings taxed? Does the winnings have an impact on other government benefits?
A: Polk County will withhold Federal and State income taxes per regulatory guidelines. These funds are considered income and must be reported to any applicable state or federal unemployment or subsidy program.
