Building a Community Health Improvement Plan for Polk County

The Polk County Health Department invites you to learn about the results of our Community Health Needs Assessment and help shape the future of health in our community. We invite residents, local organizations, and community leaders to join us as we discuss key health challenges and opportunities in Polk County.  

Join us at one of the following events to share your input! 

• Tuesday, May 6 | 5 - 6 p.m. | Virtual  
• Thursday , May 8 | 10 - 11:30 a.m. | In-Person, Polk County River Place 
• Tuesday, May 13 |10 - 11 a.m. | Virtual 

Your voice will help guide the development of our Community Health Improvement Plan (CHIP) — a roadmap for addressing the most pressing health needs in our community. By participating, you’ll have the chance to share your experiences, voice your concerns, and identify solutions that promote a healthier, more equitable Polk County.
