Community Health Needs Assessment

2024 Community Health Needs Assessment

The 2024 Community Health Needs Assessment (CHNA) is a systematic, data-driven approach to determining the health status, behaviors, and needs of residents in Central Iowa. This information helps to inform decisions and guide efforts to improve community health and wellness.

The CHNA serves as a tool toward reaching three basic goals:

  • To improve residents’ health status, increase their life spans, and elevate their overall quality of life.
  • To reduce the health disparities among residents.
  • To increase accessibility to preventive services for all community residents.

In Good Health: 2025-2027 Central Iowa CHNA Regional Plan

The 2024 CHNA identified several social determinants of health as high priorities that need the attention of our community. Information from the CHNA surveys and other relevant data sources have shown a significant increased need among central Iowa community members in the areas of housing, food security, and financial stability. These conditions are felt not only at the individual/family level, but have an impact at the community level as well.
