RFP No. 232-2425 Occupational Therapist Services for Older Adult Home..
On behalf of the Polk County Board of Supervisors and Housing Services Division, the Polk County Procurement Division will receive proposals for an Occupational Therapist that will work with Polk County Housing Staff in the Older Adult Home Modification Program-Aging in Place in determining low-cost, high impact home modifications to assist older adults with aging in place within their home. There will be approximately 200 projects within the next three (3) years.
