Community Health Needs Assessment for Central Iowa, 2024

Help make our community a healthier place and take our survey!

Multiple health organizations and community partners have joined forces to launch this Community Health Needs Assessment (CHNA). Our goal is to learn about the health needs and priorities of residents in Central Iowa.

The CHNA survey will ask you questions about your health concerns, experiences and goals. By understanding the unique challenges and opportunities you face, we can better tailor our programs and services to address the most pressing health issues our community is experiencing.

Topics covered in the survey include access to health care, chronic disease management, mental health, substance abuse, basic needs of health and more. 

Please keep an eye out for the CHNA Report come early Fall. This report will contain major takeaways and findings from the survey.

Q: How long will the survey take to complete?

A: It will take around 10 minutes to complete.

Q: Will my information be shared?

A: Your personal information will not be shared.

Q: How will the results be used? 

A: The results will be used to inform health programs and services. The results will also help us use our current resources more effectively.
