Following completion of the request there is a mandatory 5-day waiting period before the replacement title can be issued for most application processes.

Owns vehicle

Request a replacement title when requestor owns vehicle.

If you own your vehicle and need to request a replacement title, follow these steps.

  1. Complete the Application for Replacement of Iowa Certificate of Title to a Motor Vehicle, form 114033.
  2. All owners sign the completed form.
  3. Submit form following instructions listed below.

Title not yet received from lender

Request a replacement title when you no longer have payments on the vehicle, but have not received the title from the lender.

If you have completed paying for the vehicle, but didn’t receive the title from the lender/security interest holder, the lender must complete the following steps. This will allow the treasurer’s office to record the ownership status.

  1. Lender completes a separate notarized Cancellation of Security Interest., form 411168, OR
  2. A notarized letter from the security interest holder on their letterhead listing the following information is acceptable.
    • Make of the vehicle.
    • Year of the vehicle.
    • Vehicle identification number, also known as the VIN.
    • Names and address of all owners.

Loan on vehicle

Request a replacement title when there is a loan on the vehicle

If you have a loan on your vehicle and are making payments to another party, they hold the title. Most often the process to replace the title is complete by the lender/security interest.

      1. Lender/security interest completes the Application for Replacement of Iowa Certificate of Title to a Motor Vehicle, form 411033.
      2. The lien or security interest holder must sign the application.
      3. The title will be processed and mailed to the lienholder the next business day.
      4. Submit form following instructions listed below.

Submit Application

Submit the application and supporting documents to the Polk County Treasurer’s Office by one of the following methods.

  • Email vehicle@polkcountyiowa.gov
    • Attach the completed application form signed by all
    • Provide a daytime telephone number to be contacted for payment.
    • If applicable, request the lienholder send the notarized lien release.
      • The notarized lien release must be emailed directly by the lender/lienholder. The owner may not forward the notarized lien release.
    • U.S. Mail
      • Completed application form signed by all owners.
      • Original (not a copy) notarized lien release, if required.
      • $35.00 check or money order for the title replacement fee to:

  • Drop Box
    • Completed application form signed by all owners.
    • Original (not a copy) notarized lien release, if required.
    • $35.00 check or money order for the title replacement fee.
    • Secure all required documents in an envelope and place in the drop box located at the south entrance of the Polk County Administration Building, 111 Court Avenue.
