Iowa Department of Transportation Forms

Affidavit of Nonuse
Use this form if the license plates have not been surrendered for storage, registration fees have not been paid for more than three years, and the vehicle has not been operated or moved upon the highway during that period of time. If these conditions are met, the owner of the vehicle may file a statement of non use with the county treasurer. The vehicle may then be registered upon payment of the current year registration fee.

Certificate of Trust
A "Certification of Trust" is a document that is simply a condensed version of a trust. A title application in the name of a trust and the transfer of ownership of a vehicle already titled to a trust are both required to be accompanied by a copy of the entire trust agreement. In lieu of requiring a copy of the entire trust, a "Certification of Trust" can be accepted.
Heavy Highway Vehicle Use Tax Return - IRS Form 2290
Use this form to calculate and pay tax due on highway motor vehicles used during the period with taxable gross weight of 55,000 pounds or more. A copy of the submitted form is required when renewing the registration on your heavy highway vehicle with your county treasurer.
Heavy Highway Vehicle Use Tax Return (Internal Revenue Service Form 2290)

Leased Vehicle Worksheet - in state of Iowa (Department of Revenue Form 35-050)
Description: The sale or lease price of vehicles subject to registration is exempt from sales tax. These vehicles are subject to a one-time registration fee equal to 5% of the sales or lease price. This one-time registration fee also applies when vehicles are sold between private individuals.
